Matthew is one of the Others and presumably lives with them in their small community in the interior of the island. He appears to have some medical experience.

Matthew was part of the group of Others who captured Michael and stayed at the decoy village.

Season Three

Matthew was part of the group of Others present on Alcatraz island during the time when Jack, Kate and Sawyer were held prisoner there. He was also part of Ryan's team who raided the beach to kidnap Sun and the other women who may have been pregnant. However, the survivors had set a trap for the Others and Matthew was among the seven killed.
