Human Error is the twenty-fourth episode of the third season of House, M.D., and the seventieth episode overall.

Plot Overview
A married couple come from overseas via boat specifically to see House because the wife is sick. On their way over, they almost drowned and are saved by The Coast Guard, but lose the wife's medical records. As the team tries to figure out what's wrong with her, her heart stops beating although she can still talk. After keeping her alive via bypass, which House prolonged to unsuccessfully try and figure out what was wrong with her, House takes the husband to say good-bye to his wife before they pull the plug. When House pulls the plug, the wife's heart beats on its own, and she is alive. Confounding House, he finally comes to the conclusion that the problem was a genetic defect in her heart, which he finds and is fixed via surgery.
While the case is being handled, Wilson tells House how he can't accept change, pointing out how he plays the same guitar he had since eighth grade. House later fires Chase, citing a need for change as the reason, with the fact that he had been there the longest and has learned everything he could. After work, Chase is eating at a bar where Cameron joins him. Here, he tells her that his 'thing' for her was silly, and she tells him she will miss him. Later on, Foreman is on his way out when House stops him and tells him that he doesn't want Foreman to go, because he needs him. Foreman responds by saying that he doesn't need House, and leaves as planned. With 2 of his employees already gone, Cameron hands House her resignation as well, leaving House with none of his employees. The episode ends showing House getting a package at his apartment which is a new guitar.

- Chase: Chase is unexpected fired by House, with House giving the reason as it's time for change.
- Foreman: Foreman officially leaves just as he said he would.
- Cameron: Cameron gives House her resignation.
- Chase and Cameron: Cameron and Chase kiss at the end, hinting that Cameron finally accepted a romantic relationship with Chase.
- House: After Wilson tells House that he can't accept change, he fires Chase and gets a new guitar to replace the one he had since eighth grade.
